Diabetes & Heart Disease Quality Improvement with Health Systems

Client: Idaho Department of Health and Welfare

Summary: As a contractor for a state public health program, One Health Insights provides expertise and support to partner health systems to implement clinical guidelines and evidence-based interventions related to diabetes, hypertension, and cholesterol control. Through this work, we provide technical assistance support for agencies to better understand their Electronic Health Record (EHR) data and to drive and monitor their outcomes for quality improvement.

Diabetes Prevention & Management

Client: Spanish Peaks Regional Health Care

Summary: Implementing a CDC-recognized National Diabetes Prevention Program (NDPP) and an accredited Diabetes Self-Management Education and Supports (DSMES) program can be a daunting task – especially for health centers with limited resources. At One Health Insights, we pride ourselves on our ability to work collaboratively with organizations to provide the technical assistance needed for organizations to establish a host of evidence-based programming, including NDPP and DSMES.

Self-Measured Blood Pressure Monitoring Program

Client: Las Animas Huerfano Counties District Health Department

Summary: Our organization excels at developing and leading successful collaborative projects. One such example has been the implementation of a multi-partner Self-Measured Blood Pressure Monitoring (SMBP) Program which includes a local public health agency, primary care offices, hospitals, and a home health agency. Since the outset of the project, over 130 patients have completed SMBP monitoring with an average decrease in systolic and diastolic BP of 12.1 mmHg and 4.8 mmHg, respectively.
